A Google Form QR is just a URL in a square. The form already has a responder link; the QR is how someone opens it with dirty hands, no keyboard, and no bookmark.

That is why 3PLs stick them on dock doors (vehicle check), cages (damage report), and visitor badges. Same three steps as a café feedback form.

1. Copy the responder link

In Google Forms: Send → link icon → copy. It looks like https://docs.google.com/forms/d/e/…/viewform.

Do not copy the edit link (/edit). That URL is for you; scanning it on a shared warehouse phone is a bad day.

Shorten only if you must print tiny. A static QR of the full Google URL is fine at 30 mm+. If you expect to replace the form later (new version, new season), use a dynamic QR so the printed sticker stays and you change the destination.

2. Generate the code

Paste the viewform URL below. Download PNG for laser/inkjet or SVG for a print shop. Leave a quiet zone (blank margin) around the code.

3. Print where the work is

  • Laminate if the dock gets rain
  • Test with the shared Android, not only your phone
  • Add a four-word caption (“Scan to log damage”) — QR without a verb gets ignored

  • Goods-in checklist with required photo questions
  • Near-miss / incident so agency staff do not hunt a supervisor’s email
  • Visitor / contractor sign-in on a tablet stand
  • Client audit walk: scan bin QR, then a form for empty/damage/expiry exception (pair with rack labels)

Forms capture answers. They do not capture how many people scanned and bounced. If you need that, save the code as dynamic in a workspace and keep the same print.

iPhone, Android, shared devices

Both camera apps open the URL in the browser. On a shared scanner-phone, sign out of the last person’s Google account or use “incognito” / a dedicated user so responses are not attached to a random Gmail.

If someone sends you a photo of the form QR instead of the link, decode the picture and confirm it is the viewform host before sharing it further.

Does a Google Form QR expire?
The QR is only as durable as the link inside it. A static QR never ‘times out’, but if you delete the form the scan opens a dead page. Keep the form; or use a dynamic QR so you can swap the URL.

Google Forms records responses, not scans. Use a dynamic QR if you need scan counts by location or device.
Will it work on iPhone and Android?
Yes. Phone cameras open the form URL in the browser. Sign respondents out of the wrong Google account on shared warehouse devices.
